PMHScribe listens during your virtual session and generates a complete, sign-ready psychiatric note in seconds. No dictation. No catch-up charting.
No credit card required.
Allison Sikorsky, DNP, PMHNP founded her multistate telepsychiatry practice as a platform in 2018. As Chairperson of the American Telemedicine Association’s TeleMental Health Special Interest Group and a speaker on Ethical AI in Telemedicine, she brought that clinical reality into every feature PMHScribe builds.
She built PMHScribe because no existing AI scribe understood psychiatric documentation: not the Mental Status Exam, not psychotherapy add-on notes, not asynchronous telehealth workflows. Generic scribes were built for primary care. PMHScribe was built from her own clinical workflow, which means it captures what psychiatrists and PMHNPs actually document.
Before becoming a PMHNP, Allison worked in telephone triage and asynchronous care delivery, when clinical notes meant a typist listening to dictaphone cassettes and typing up paper charts. She has been solving the remote care documentation problem longer than most AI scribe companies have existed.
No voice commands. Just see your patient the way you always have.

Open PMHScribe before your telehealth session and tap record. It runs in the background on any device with a microphone.

Focus entirely on your patient. PMHScribe listens for MSE findings, medication discussions, risk factors, therapy interventions, and clinical decisions as they happen.

When the session ends, your note is ready: structured, accurate, and easily formatted for you. Review, edit if needed, and paste. Done before your next patient joins.
Every element a telehealth psychiatric note requires.
Appearance, behavior, speech, mood, affect, thought process, thought content, cognition, insight, and judgment. Every telehealth note includes a complete MSE, not a generic summary.
PMHScribe generates a separate psychotherapy note alongside your E/M. Billed separately per CMS rules. Most scribes cannot do this.
PMHScribe documents patient education on medications in the note: what the medication is for, expected onset, side effects reviewed, monitoring parameters, and patient questions addressed. No other AI scribe does this. Required for audit-readiness.
PMHScribe captures suicidal ideation (SI) and homicidal ideation (HI) — including suicidality and homicidality language — directly from your verbal assessment. The generated note includes an Imminent Risk Assessment section with safety planning, protective factors, and clinical decision documentation. Every session, every time.
Zoom, Doxy.me, SimplePractice, TherapyNotes, or any HIPAA-compliant video platform. No integrations required. PMHScribe currently uses a copy-and-paste workflow
Your telehealth notes stay in PMHScribe indefinitely. Access, search, and reference any note you have ever created. Competitors auto-delete. PMHScribe does not.
PMHScribe documents asynchronous telehealth workflows that synchronous-only scribes ignore entirely. Portal message E/M visits (Online Digital E/M) are documented as billable encounters. In-person exemption requirements for Medicare telehealth patients are captured in the note. Remote order documentation is built in: EKG orders, lab orders, and other orders sent to patients without on-site lab access. Built by a clinician who has run an asynchronous telehealth platform since 2018.

Psychiatrists and PMHNPs practicing telehealth spend 2 to 4 hours every night finishing notes after clinic. It has a name: pajama time. Virtual practice eliminated the commute but not the charting.
PMHScribe was built to fix that. PMHScribe currently uses a copy-and-paste workflow. Hundreds of psychiatrists and PMHNPs have already stopped charting at night.
Yes. PMHScribe works with any telehealth platform — Zoom, Doxy.me, SimplePractice, TherapyNotes, or any HIPAA-compliant video tool. No integrations or software changes are required. PMHScribe runs in your browser and captures audio from your device microphone during the session.
Yes. PMHScribe generates a complete Mental Status Exam for every session, including appearance, behavior, speech, mood, affect, thought process, thought content, cognition, insight, and judgment. This is captured from your verbal session, not entered manually. Generic AI scribes often omit or abbreviate the MSE because they were not built for psychiatry.
Yes. PMHScribe generates separate psychotherapy add-on notes for 90833, 90836, and 90838 alongside your E/M note. These are documented separately per CMS billing rules. This is a feature unique to PMHScribe — most AI scribes generate a single combined note that does not support add-on psychotherapy billing.
Yes. PMHScribe is fully HIPAA compliant. A signed Business Associate Agreement is included with every paid plan at no additional cost. Audio is not retained after transcription. Only the generated chart note is stored, and notes are retained until you choose to delete them.
Yes. PMHScribe works for any licensed mental health prescriber or therapist regardless of the state where the patient is located. NPI validation at signup confirms licensure. PMHScribe is used by multistate telehealth psychiatrists, PMHNPs, and therapists across the US.
PMHScribe was built by a PMHNP who practices telehealth psychiatry, not by a general medicine AI company. It generates a complete MSE, separate psychotherapy add-on notes, medication education documentation, and Imminent Risk Assessment capturing suicidal ideation (SI), homicidal ideation (HI), safety planning, and protective factors — features that generic scribes do not include because they were not trained on psychiatric workflows.
Yes. PMHScribe supports asynchronous telehealth workflows including Online Digital E/M for portal messages. It also documents in-person exemption requirements for Medicare telehealth patients and supports order documentation — including EKG and lab orders — for providers treating patients remotely without on-site lab access.
Notes are generated within seconds of ending your session. The note is structured, formatted, and ready to paste into your EHR. Most providers review and sign in under 2 minutes. PMHScribe eliminates post-session charting, which is the primary driver of pajama time in telehealth practice.
Your last telehealth session ends your day. PMHScribe handles the note.
