Interactive Complexity is a key billing add-on in CPT 2025 that reflects the extra time, skill, and effort required when specific communication barriers or emotional dynamics arise during a mental health encounter. This add-on can be reported in addition to the following CPT codes:

90791 – Psychiatric diagnostic evaluation (no medical services)
90792 – Psychiatric diagnostic evaluation with medical services
Psychotherapy codes – 90832, 90833, 90834, 90836, 90837, and 90838

According to the American Medical Association CPT 2025, there are four specific reasons to bill interactive complexity. Below, we’ll break down each reason—with real-world behavior that providers often encounter and what makes the encounter “more complex than usual.”

1. Maladaptive Communication That Complicates Care

This occurs when the patient’s behavior or communication disrupts the delivery of psychiatric or therapeutic services. Examples include:

High anxiety leading to panic attacks mid-session
Reactivity such as shouting, yelling, or crying
Argumentative or drug-seeking behavior around diagnoses or treatment plans
Perseverative questioning, especially around controlled substances
Provider needs to de-escalate the patient to proceed with care

In short: if the session required more effort to maintain clinical engagement due to the patient’s communication style, this modifier likely applies.

2. Caregiver Emotions That Impede Treatment Implementation

In sessions involving parents or other caregivers, sometimes the caregiver—not the patient—adds complexity. Situations may include:

High caregiver anxiety, preventing clear discussion of the treatment plan
Repeated questioning or interrupting, making it hard to deliver care
Disagreements about the diagnosis or recommendations that impair the ability to deliver care
Caregiver disbelief or resistance, derailing the conversation

When the provider assists the caregiver’s emotions or behavior to keep the session productive and moving forward with treatment, beyond psychoeducation, this too qualifies as interactive complexity.

3. Disclosure of a Sentinel Event or Mandated Report

A sentinel event, such as abuse, neglect, or serious trauma, that arises during the session requires more time, documentation, and action:

Mandated reporting to child or adult protective services (e.g., DCFS)
Discussion of abuse disclosure with the patient or caregiver
Complexity in managing emotional fallout after such disclosures
Documenting and preparing a written or verbal report to authorities

This adds substantial complexity to the visit and should be captured appropriately.

4. Use of Play Equipment or Communication Devices

Some sessions require nonverbal or assisted communication methods to engage the patient:

Developmental delays mean the patient has not yet acquired expressive language
Loss of language skills, such as in regression or trauma
Difficulty describing symptoms, emotions, or treatment response verbally
Lack of receptive understanding, requiring visual, sensory, or play-based aids

If the provider uses tools like puppets, drawing, dolls, picture boards, or sensory items to facilitate therapeutic interaction, this is interactive complexity.
