Built by a Clinician

Built for Telemental Health. Not Adapted From Primary Care.

Allison Sikorsky, DNP, PMHNP founded her multistate telepsychiatry practice as a platform in 2018. As Chairperson of the American Telemedicine Association’s TeleMental Health Special Interest Group and a speaker on Ethical AI in Telemedicine, she brought that clinical reality into every feature PMHScribe builds.

She built PMHScribe because no existing AI scribe understood psychiatric documentation: not the Mental Status Exam, not psychotherapy add-on notes, not asynchronous telehealth workflows. Generic scribes were built for primary care. PMHScribe was built from her own clinical workflow, which means it captures what psychiatrists and PMHNPs actually document.

Before becoming a PMHNP, Allison worked in telephone triage and asynchronous care delivery, when clinical notes meant a typist listening to dictaphone cassettes and typing up paper charts. She has been solving the remote care documentation problem longer than most AI scribe companies have existed.

How It Works

Three Steps. Session to Signed Note.

No voice commands. Just see your patient the way you always have.

Start Recording

Open PMHScribe before your telehealth session and tap record. It runs in the background on any device with a microphone.

See Your Patient

Focus entirely on your patient. PMHScribe listens for MSE findings, medication discussions, risk factors, therapy interventions, and clinical decisions as they happen.

Get the Note

When the session ends, your note is ready: structured, accurate, and easily formatted for you. Review, edit if needed, and paste. Done before your next patient joins.

Features

What PMHScribe Documents During Telehealth

Every element a telehealth psychiatric note requires.

Mental Status Exam

Full Mental Status Exam

Appearance, behavior, speech, mood, affect, thought process, thought content, cognition, insight, and judgment. Every telehealth note includes a complete MSE, not a generic summary.

Psychotherapy Add-Ons

Psychotherapy Add-On Notes (90833 / 90836 / 90838)

PMHScribe generates a separate psychotherapy note alongside your E/M. Billed separately per CMS rules. Most scribes cannot do this.

Exclusive Feature

Medication Education

PMHScribe documents patient education on medications in the note: what the medication is for, expected onset, side effects reviewed, monitoring parameters, and patient questions addressed. No other AI scribe does this. Required for audit-readiness.

Risk Documentation

Imminent Risk Assessment

PMHScribe captures suicidal ideation (SI) and homicidal ideation (HI) — including suicidality and homicidality language — directly from your verbal assessment. The generated note includes an Imminent Risk Assessment section with safety planning, protective factors, and clinical decision documentation. Every session, every time.

Platform Compatibility

Works With Any Telehealth Platform

Zoom, Doxy.me, SimplePractice, TherapyNotes, or any HIPAA-compliant video platform. No integrations required. PMHScribe currently uses a copy-and-paste workflow

Note Retention

Notes Retained Until You Delete Them

Your telehealth notes stay in PMHScribe indefinitely. Access, search, and reference any note you have ever created. Competitors auto-delete. PMHScribe does not.

Asynchronous Telehealth

Asynchronous Telehealth Documentation

PMHScribe documents asynchronous telehealth workflows that synchronous-only scribes ignore entirely. Portal message E/M visits (Online Digital E/M) are documented as billable encounters. In-person exemption requirements for Medicare telehealth patients are captured in the note. Remote order documentation is built in: EKG orders, lab orders, and other orders sent to patients without on-site lab access. Built by a clinician who has run an asynchronous telehealth platform since 2018.

Telehealth Was Supposed to Give You Your Life Back.

Psychiatrists and PMHNPs practicing telehealth spend 2 to 4 hours every night finishing notes after clinic. It has a name: pajama time. Virtual practice eliminated the commute but not the charting.

PMHScribe was built to fix that. PMHScribe currently uses a copy-and-paste workflow. Hundreds of psychiatrists and PMHNPs have already stopped charting at night.
